Enrollment Packets are available to be picked up at school now!!

Forms may also be printed online by using the links below. Forms must be printed and cannot be completed online at this time.

Please fill out your enrollment packet beforehand to ensure a quick and easy enrollment. Remember some forms must be signed by the parents and the students. Individual Packets must be filled out for each student.

 All 4th -12th grade students who are planning on participating in sports this year will need to have a physical completed.

ALL 7th grade students are required by the State of Oklahoma to have a tdap booster to attend school. Please bring an updated copy of your child’s immunization records when you receive any immunizations.

ALL OF LAST YEAR’S OUTSTANDING BILLS MUST BE PAID IN FULL BEFORE YOUR CHILD CAN ENROLL THIS YEAR.
